I’m ready to say goodbye to my gap… what are my options?

I’ve had a gap between my front teeth for as long as I can remember. I’ll be turning 40 soon and I’d like to start this next phase of my life by getting it fixed. My dentist recommended braces, but I’d like to explore other options that might not take as long to see a result. His other option was porcelain veneers, which seemed ok at first, but now that I have learned more about the procedure, I am not sure about having my teeth shaved down. So now I am back to braces unless there is another alternative. Can you help?  — Felicia in Arkansas

Kudos to you for reaching such a milestone and wanting to look and feel fabulous. The good news is that you have a couple other choices in addition to the two your dentist mentioned that will get you the result you desire:

  • Bonding – If you’re simply looking to fill in a small gap (and your dentist is skilled in bonding) he can add a tooth-colored material to either side to fill it in. Bonding is less invasive and requires less tooth preparation than veneers.
  • Six Month Smiles – This procedure utilizes clear braces to correct aesthetic issues, often producing results in as little as four months. It’s a great choice if you only need to correct the appearance of your teeth and don’t need any adjustments to your bite. While they are a great alternative to traditional braces, many dentists are not experienced with Six Month Smiles. You may have to search for an experienced dentist and they will have to evaluate whether or not you are even a candidate.

Definitely do your research and don’t be afraid to seek out a second opinion when considering all your options. Regardless of what method you choose, you’ll get beautiful results. However, Six Month Smiles will be the least-invasive – and quickest way to help you achieve that “fabulous at 40” smile!

Does teeth grinding affect Lumineers?

I’ve always been self-conscience of my smile and I’m finally to the point financially where I am considering getting Lumineers. I’ve seen and heard a lot of good things about them, but my one concern is that I grind my teeth. I’m afraid if I tell my dentist that I grind my teeth, he will refuse to put them on. If I don’t tell him, will I risk them failing? I’d be willing to replace them down the road, but can’t afford to do it if they get damaged right away. No matter what type of medical advice you are seeking, it’s always best to be honest. Grinding your teeth can be a problem with Lumineers, but whether or not it totally excludes you from the procedure is something your dentist will need to determine.

Grinding and clenching can affect the longevity of Lumineers, but it’s almost impossible to determine ahead of time if or when they’ll fail. If you grind your teeth during sleep, a night guard can be a great solution for you.  Ask your dentist about other appliances that can help you during the day or even seek out some relaxation techniques designed to help you reduce the urge to grind.

There are some other alternatives to Lumineers that may hold up better in your situation. They will give you the same beautiful look, so be open to suggestions from your dentist on other brands or styles.

Also, keep in mind that no dental restoration will last forever. Whatever you choose will eventually need replaced. Any restoration can fail in a very short period of time if they are not properly taken care of. But, when correctly placed and cared for, restorations can last ten years or more. In your case, honestly is the best policy… and so is a frank discussion with your dentist about your situation and the results you would like to achieve. When all is said and done, you’ll be glad you did when you can show off your beautiful, new smile to the world.

What can I do to cover tetracycline stains?

I have had gray stained teeth as a result of tetracycline for many years. All I really want is to have nice, white teeth. I’ve seen and heard a lot of negative stories about porcelain veneers and Lumineers. I’ve also researched bonding. What procedure would give the best results for my situation? — Adrianna in Tennessee

Tetracycline stains the teeth from the inside out and are very difficult to remedy – particularly with traditional teeth whitening. The best procedure to mask the stains is probably porcelain veneers. While Lumineers might be a cheaper option, keep in mind that they are very thin and the dark enamel might show through. You’ll have similar problems with cosmetic bonding, plus bonding is also susceptible to staining and it is not as durable as porcelain veneers.

Porcelain veneers will give you the best result because they will completely mask  and cover the dark enamel. What’s even better is that the procedure is much less invasive compared to other treatments (i.e. porcelain crowns) and if you have them done by an expert cosmetic dentist,  you shouldn’t have any pain or sensitivity. The result will be a beautiful, natural-looking smile you will be proud to show your family and friends.

Unhappy with my porcelain veneers… shouldn’t my dentist be required to fix them?

I am wondering what I should about the porcelain veneers I recently had placed… I am so unhappy with them. I can see two problems with them: 1) my canine teeth look crooked and 2) I noticed a little line above my left front tooth and wonder if it is my natural tooth surface showing through. When I went back to my dentist with my concerns, he refused to do anything about the line because he said it was barely noticeable. I spent so much money and now I am afraid that area might decay. I chose porcelain veneers because I had old bonding that needed to be replaced, but now I am just as insecure with my smile as I was before. Is it really not a big deal and I am just being difficult? It sounds like you have a couple of things going on here. To address the issue of your dissatisfaction with how your smile looks, it is quite disappointing that your dentist didn’t do a better job of letting you see what your smile was going to look like before permanently bonding it. Good cosmetic dentists want to always make sure you see exactly how your porcelain veneers will look, and will even let another family member or a friend look at them with you. They may even offer a beautiful smile guarantee because their first priority is making sure you absolutely love your new smile.

As a patient, you have a right to ask him to re-do what you don’t like… particularly the crooked canine teeth. As far as the line at your gumline, unless there is a groove or ledge that could “catch” decay, you tooth is not going to be vulnerable, so he does have a point there.

The other issue at hand is the difference between a true cosmetic dentist and a general dentist who “likes” to do cosmetic dentistry. A good cosmetic dentist understands: 1) that he or she is not just treating a patient, they are treating the self image of the patient, and 2) if the patient is unhappy, he or she has failed. Good cosmetic dentists will want you to leave the office absolutely loving your smile, and if you don’t they will make any and all adjustment needed to achieve that end.

Can I whiten my teeth inexpensively at home?

A few years ago, I was able to save up so I could afford an in-office bleaching procedure. I was very happy with the results, but I really can’t afford to do it again. I’d love to find a cheaper alternative that I could do at home, but will I be able to achieve the same results?

Just imagine living in during 17th or 18th century where teeth whitening meant saving down your teeth and bathing them in acid. Thankfully, for us, teeth whitening has come a long way since then and you now have all sorts of options to choose from depending on your budget, how quickly you want to get the job done or how white you want to go.

Today, most dental offices offer custom-made whitening trays. While you’ll get similar results as the in-office treatments, it takes longer. The trays are yours to keep for later use, so that means only a one-time expense as long as you take good care of them and there are no changes to your mouth. The only thing you need to do is purchase the whitening gel from your dentist as needed.

If you’re lucky enough, some dentists offer free bleaching trays and gel on a regular basis at your first visit as in incentive to continue with routine cleanings and check-ups.

Why do my gums look gray around my implants?

I recently had a couple of implants placed, but I think there is a problem and it is very noticeable – the gums around the implants look gray! My dentist didn’t mention anything about this, and now I am worried that I paid a bunch of money and something is wrong with my implants or the materials that were used. Should my gums be turning gray? — Sheila in New Orleans

The grayish or dark color is likely the collar of the implant showing through your gum tissue. This is not uncommon and it can be a result of your gum tissue being too thin, as well as the implant not being placed at the right depth. It is the titanium from the implant visible at the gum line, which can eventually lead to gum recession.

You could have a soft tissue graft done to thicken the surrounding gum tissue or if you haven’t had your permanent crown placed yet, your surgeon could switch out the titanium for Zirconia (a tooth colored material that would eliminate the gray color). Regardless of what you decide, you definitely need to have your dentist take a look at it and make sure there is no bone loss.

There’s no doubt that implants are a major investment. All too often, people look for ways to cut costs or make implants more affordable. Many times that means compromising on quality and unfortunately there are dentists who cut corners or costs in the materials. Find an implant dentist who is properly trained and has the credentials and experience needed to give you the best results possible.

A great deal may be good for some things in life, but not when it comes to dental implants. The “deal of a lifetime” is likely too good to be true and could end up costing you even more money and heartache in the end.

I’ve had it with dentures that don’t fit!

I’ve had my dentures now for about two years and I am just sick of them! Even when I have them adjusted by my dentist (which is quite frequently) my mouth is still sore and what’s even worse, I can hardly speak or eat. I’m ready to throw them in the garbage and just forget about them because I am pretty sure my dentist is tired of me complaining all the time. He told me that it won’t get any better and I should just get used to it. Do I really have to live with ill-fitting dentures for the rest of my life?  — Randy in Illinois

It certainly is no fun to have dentures that interfere with your daily activities and affect your quality of life. There is a procedure that can help and it’s called “all-on-fours.” Your dentist may not have mentioned it to you because it’s a procedure he may not do.

All-on-fours are dental implants that help support your denture. If this is something that interests you, find a dentist who is knowledgeable in implant-supported dentures who can place four individual implants that will evenly distribute the pressure from the denture. All-on-fours are a long-term solution that will provide better security, restore your ability to talk and eat without the fear of your dentures falling out, and even better, they can stop or at least slow the jaw bone shrinkage that happens when you lose all your teeth.

Ask your dentist about all-on-fours. If he is unable to do them, he can refer you to a specialist who can help determine if they are right for you.

Do porcelain veneers yellow?

Over the years, my teeth yellowed and I became very self-conscious about it. I saved up and recently had porcelain veneers put on, but realize that I never asked my dentist if my veneers will eventually start to discolor or yellow. I’ve spent way too much money to end up with ugly, yellow teeth again in a few years. Will my veneers yellow?

This is a great question and one that is asked quite often. Your porcelain veneers will not yellow, but that is not to say that veneers can’t take on a discolored appearance if the natural tooth they are bonded to picks up stains. If your teeth are susceptible to stains from coffee, dark sodas, berries, etc, your best bet is to avoid those things. And you should also avoid smoking, too.

Also, get rid of any whitening toothpastes you may have used in the past. While you want to keep your teeth white, these toothpastes are highly abrasive and can actually damage your veneers by breaking down the glaze that is designed to keep them white.

If you do consume any foods or drinks that stain your teeth, a good practice is to rinse your mouth immediately after eating or drinking. This will help assure you have that bright, beautiful smile for many years to come.

Can my 16-year-old get porcelain veneers?

My daughter’s front teeth are somewhat discolored, crooked, and one is larger than the other. She is only 16, but she has been begging me to let her get porcelain veneers. Is she too young?

There is no set minimum age for veneers; however there are some risks when placing veneers on younger patients if they have not yet reached maximum jaw maturity. If place too early, the veneers may need to be redone a few years later if she experiences a growth spurt.  The bones and gums continue to develop as we age, and that can cause the porcelain edge of veneers placed too early to become exposed. Your 16-year-old may be a good candidate for veneers – it really depends on the condition of teeth, how crooked they are, and if they are fully developed. If you and your daughter fully understand that the veneers will likely need replaced once or twice later down the road, the decision is really more of a personal choice.

What Can I Expect When Getting Dentures?

I think I am to the point where I need to get dentures. How does the procedure work? Will my dentist put me to sleep and pull out all my teeth and make the dentures all at one time?

If you are in need of dentures, your dentist will extract all of your teeth at one time. Most patients have the option of IV or oral sedation, but if you want to just have local anesthesia and be awake for the procedure, you can do that, too. Keep in mind that not all dentists are certified in IV and oral sedation, so if this is something you prefer, be sure your dentist offers it.

When it comes to dentures, you have several options. An immediate denture is the most common type of denture and it is placed right after your teeth are extracted so you can leave the office with a new smile. Another benefit of an immediate denture is that it helps reduce swelling and bleeding. They are, however, more expensive than conventional dentures because they take more time to make and you will need more follow-up visits for adjustments after initial placement.

Conventional dentures are placed about eight weeks after healing, but the advantage is that you will likely have a better fit then with an immediate denture. During the healing process your gum tissue shrinks and the immediate becomes loose, requiring a soft temporary reline material to be placed on the denture for refitting. After about six months, when your mouth is completely healed, you will need a more permanent reline or possibly a new denture.

Before you make any decisions, talk with your dentist or specialist and discuss all your options. Dentures don’t always stay put in the mouth, and that is a problem for some patients. There are options to make dentures more permanent, such as snap on dentures that snap onto dental implants. Be sure to learn as much as you can before determining the type of denture that is right for you.
