Dealing with Teeth Whitening

I developed these opaque white spots on my teeth when I was much younger. My dentist insists my teeth are completely healthy. I mentioned to him that the appearance really bothers me, but he is more of the philosophy if it isn’t broke don’t mess with it. When I brought it up a few more times he said if it bothers me that much I could get some whitening strips. I’m looking for something a little faster and professional. There is a dentist in my area who does Zoom Whitening. Is it okay to go to a different dentist just for some cosmetic work or is that considered bad form?

So, there are a few different types of dentists. Yours sounds like he holds to the engineering mindset These are dentists who got into the field because they like to fix things. They haven’t invested any time in learning the cosmetic side of their field. This is very obvious from the advice he gave you.

While you are correct that Zoom Whitening is the most effective and quick way to get your teeth whitened, it will not work for your particular issue.

Any type of teeth whitening, professional or not, will whiten your teeth uniformly. This means there will still be color differences in your teeth with white spots.

The Solution to White Spots

Depending on the area affected, you have a couple of choices. If it is a tiny area of white spots, you could have dental bonding done to get your teeth uniform in color.

If it is a large area, you would be better served getting porcelain veneers placed. These will cover the entire tooth and are the top of the line in smile makeover options.

I have one tooth which has had two white spots on it my entire life, or at least for as long as I can remember. I like the white parts and want to get all my teeth to look that way. My dentist is suggesting I get Zoom Whitening but my sister says that won’t work that the spots will still be whiter than the rest. Who’s right?

Your sister is correct. It worries me that you’re getting better advice from your sister than you are your dentist. He is supposed to be the professional.

Teeth whitening whitens teeth uniformly. This means that all the natural tooth structure will whiten, including the white spots. That doesn’t mean there isn’t a solution.

You can go ahead with the Zoom Whitening. This will get your teeth as white as possible in just one appointment. You’ll still have those two spots, but they can be fixed with either dental bonding or a porcelain veneer. Dental bonding costs less, but will have to be replaced a little more often.

White Spots on Teeth

Sometimes there are white spots on teeth which are just the pigment. This seems to be your case because you’ve had it for as long as you can remember.

Other times, however, white spots are more serious. Usually, these are signs of decalcification and an early sign of decay. Zoom whitening made my teeth look worse!

I recently got my braces off and have very noticeable white spots on my front teeth. My dentist recommended Zoom whitening, so I tried it. I could tell right away that the spots looked even worse, but he said I just needed to wait a few days for the color to even out. It’s been two weeks and my teeth still look the same. What’s going on? I’m thinking about trying an at-home kit to see if it will work. — Sara

Dear Sara,
Your dentist seems to have steered you in the wrong direction. Did he actually examine your teeth before recommending the Zoom whitening?  It sounds like you probably have decalcification. It’s a common problem in which the minerals in the tooth are stripped away as a result of poor cleaning around braces. These areas of decalcification manifest as white spots and it make the teeth susceptible to decay.

Any dentist who is familiar with this problem or who is skilled in cosmetic dentistry, would know that Zoom Whitening or other whitening systems will make the white spots worse. There’s a chance you can add minerals back to those areas, but there’s no guarantee it will work. Products that might help are Tooth Mousse or MI Paste (have your dentist order them for you or you can purchase them online). They are formulated  to saturate the tooth in minerals, in hopes of reversing the decalcification and staving off any future decay. Some dentists will do Microabrasion on the areas, but the end result could make your teeth look creamy – not white. You could also ask about removing the spots and placing bonding on the teeth. That will keep the areas from decaying — and your teeth will be a uniform color. Your only other alternative would be to look at a more costly cosmetic procedure like porcelain veneers.

Will tooth mousse and whitening get rid of my white lesions?

I’ve had white spot lesions on my teeth for several years. I finally decided I wanted to do something about them so at my last dental appointment, my dentist sent me home with tooth mousse and then told me to come back in a couple of months for some whitening. Will the combination of these two things help me finally get rid of these stains for good? — Eric

Eric,
Unfortunately, your dentist is not quite on the mark with this one. Yes, tooth mousse does treat white spots, but it’s only designed to keep them from turning into cavities by remineralizing them. You can check into it, but the company doesn’t make any specific aesthetic claims beyond that.

But what is even more concerning is that your dentist is recommending tooth whitening. The bleaching gel is designed to whitening everything, so if you have white spots, they will end up looking even worse. Find an expert cosmetic dentist in your area and he/she will be able to help you get rid of the white spots with either microabrasion (an acid-pumice mixture that is applied to the surface of the teeth) or tooth bonding which is covering the areas with composite resin.

Zoom whitening made my white spots look worse

My teeth have always had little white spots on them, and I was hoping that having them bleached would make them less noticeable. I talked to my dentist about Zoom whitening and decided to have it done, but I am utterly disappointed with the results. Instead of hiding the white spots, they are even brighter. What happened?

The white spots sound like fluorosis – a condition that results from excess fluoride while the teeth are still forming. Zoom whitening and other bleaching products typically brighten natural tooth structure for most people, but if you have fluorosis, your dentist needs to use a different treatment.

You need to talk to your dentist in more detail about the results you were hoping to achieve. There are treatments that will hide the spots, but you and your dentist need to determine which one is best for your situation, and more often than not, it will be either using tooth-colored composite material or placing porcelain veneers. The good news is that it’s best to bleach before having restorative work done – especially if your goal was to not only hide the white spots, but overall have a brighter smile.

Also, you can expect it to take about a week for your new tooth color to stabilize, and you may even notice the white spots even less and decide that they look ok. Regardless of whether or not you seek further treatment, let your dentist know how you felt about the results.
