help avoiding dental implant failure

I’m looking at dental implants. One dentist is insisting on a CT scan but he uses a brand of implants I’ve heard a lot of good things about called Straumann. The other dentist says the CT scan isn’t necessary, but he uses a brand I can’t find much about called Southern Venturi. I like the idea of saving money on the CT scan, but want to make sure the brand is okay first. Do you have an opinion on that brand?

I’m glad you are asking these questions. Both have important implications for a successful dental implant case. Let’s start with the CT scan. I would spend the money on that. The big reason is getting dental implants is a 3-Dimentional procedure. In order to get the right placement, it helps to have 3-Dimentional scans.

A lot of dentists are willing to place the dental implants without the scan but here’s what concerns me about that. If you’re talking about a an implant for a front tooth, without the scan the dentist risks it perforating your sinuses. Some dentists try to circumvent that issue by using a small implant, but they aren’t always able to retain the tooth in the long-term.

Another important piece of information is the depth and thickness of the bone. You only get that with the CT. This one additional step can prevent a host of problems.

The Brand of Dental Implants

The first brand you mentioned, Straumann, is one of the brands the top implant dentists prefer. Other good ones are Nobel Biocare, BioHorizons, Zimmer, and Astrotech.

The second brand you mentioned has me a tad concerned. I don’t know any of the top dentists who use this brand. It may be a cheaper brand which saves the dentist money, but you have to be very careful with “cheap” implant fixtures.

Often, it goes fine at the procedure but fails later and the patient has no idea it’s because the dentist used a cheap implant fixture. You also have no real recourse if that happens. It’s much better to pay a little more and get a proven fixture.

The Importance of a Skilled Implant Dentist

Another huge factor in the prevention of dental implant failure is the skill of the dentist. You want a dentist with extensive dental implant training. You can look at some of Dr. Delaune’s credentials to get an idea of what you are looking for in a dentist.

Don’t hesitate to ask where they received their dental implant training. Dental school isn’t enough. They must have post-graduate training.

You also want to make sure they do a significant amount of cases and have a lot of experience. Additionally, ask about their success rate. I’d want someone with a 98% success rate.

Custom mouthpiece for grinding because of meds?

My dentist is trying to convince me to get this custom-fitted device to sleep with. He says I’m grinding my teeth because of the medicine I take. First, wouldn’t I know if I happened to be grinding my teeth and second, can medicines really make you do that? Does it even matter?

I promise your dentist isn’t trying to pull one over on you. Everything he said is based in fact. Most patients doen’t realize they are grinding their teeth, because it happens at night while they’re sleeping.

The Consequences of Grinding and Clenching

Griding and clenching your teeth is known as bruxism. When you do that habitually, whether intentionally or not, your teeth begin to wear down. In some cases, I’ve seen them worn down to nubs and the patient had to have all their teeth crowned. This is known as a full-mouth reconstruction and is very costly.

In addition to wearing down the teeth, you can also end up chipping or even cracking them . Cracked teeth will also have to be crowned in order to save them.

A nightguard protects your teeth from the force of these motions. Even though you likely won’t be able to stop doing them, your teeth won’t bear the consequences.

With constant motion of your jaw in grinding, you’re in danger of another kind of problem. You could wear down your temporomandibular joint, leading to TMJ disorder.

Are Lumineers the best?

I am planning on a smile makeover. I want to get the best one possible. I keep hearing good things about Lumineers. Are they as good as their advertisements say?

You are right that Lumineers are heavily advertised. They happen to be one of only brands known by name because of that. While they are okay, you can do better. Plus, there is a much more important consideration than the brand.

More Important than the Brand

While some brands are better than others, you can use the absolute best brand and the results would totally depend on the skill of the cosmetic dentist using that brand.

The skill of the dentist is the number one determining factor in whether or not your smile makeover ends up stunning or sad.

Finding an Expert Cosmetic Dentist

You want a dentist who has invested a lot of post-doctoral time in cosmetic training. Smile makeovers aren’t taught in dental school. Instead, a dentist has to intentionally take the time to study its theory and practice.

Just like not all painters have the same skill level, neither do cosmetic dentists. One dentist will have a more artistic eye than another. So, how do you know if you’re getting a Michelangelo or a Mickey Mouse?

Clorox for teeth whitening

I want to whiten my teeth fast. I know that traditional teeth whitening costs money and takes several weeks. I’d like it done faster. I was thinking if Clorox bleaches clothes, maybe it can bleach teeth too. If I put it on with a Q-tip, how fast will it work?

First, Clorox bleach is a corrosive and will burn living tissue like your gums as well as eat away at the enamel on your teeth. worried teeth whitening store damaged my teeth

I’d been considering teeth whitening for a while. My dentist is pretty pricey so I had been putting it off and slowly saving. I happened to notice a new teeth whitening shop in my mall when I was window shopping. I popped in and asked some questions.

What they offered sounded great. Their workers were trained and certified teeth whitening specialists and the cost was so much less I’d already saved enough. I asked them why it was cheaper and they said it’s because they’re not running an entire dental practice and focus on one treatment which lowers their cost.

I went the next day. I’m in absolute agony. My gums feel like they’re on fire. My teeth are dark where my fillings are so they look splotchy and my dental crown didn’t whiten at all. My smile looks ridiculous and I’m in pain. Did they damage my teeth?

It’s likely true they have less overhead, but they should have invested a least a little more in some training for their employees. First, you should know there is no such thing as a “Certified Teeth Whitening Specialist”. My guess is that’s simply an internal term they use for their employee training.

If you had the procedure done in one day in the store than it was likely something like Zoom Whitening, which can be completed in one appointment.

Your gums are in pain because they neglected to protected them from the light which jump starts the whitening agents. A dentist would have placed a special coating on your gums so they’d be unaffected by the light. Without that, you get the equivalent of a severe sunburn on your gums.

To help you alleviate the pain, you can try some over the counter pain meds and salt water rinses. It won’t take it away completely, but it should take the edge off of it.

What Went Wrong

Your teeth aren’t damaged, but you will need some additional work. They should have informed you that teeth whitening only works on natural tooth structure. That’s why your teeth look splotchy and the crown is dark.

The only way to get them to match your teeth is to have them replaced. If your crown is visible, which it sounds like it is, make sure your dentist provides you with an all-porcelain crown. They look completely natural and can match your teeth exactly when done by a good cosmetic dentist.

Deciding between traditional & ultra-thin Veneers

I’m very excited about getting a smile makeover. I don’t want to go into this blind. At the moment, I’m trying to decide between traditional veneers and ultra-thin veneers. The only thing is, i keep reading that some people who get the ultra-thin veneers end up with a bulky smile, while others a beautiful one. I can’t figure out how ultra-thin veneers can create a bulky smile. How do I know which one to choose?

It’s great that you are doing research. I’m a big believer in increasing our knowledge of everything we can. The only thing is you are researching the wrong thing.

The reason some cases of ultra thin veneers look wonderful and others don’t is the various skills of cosmetic dentists. An expert cosmetic dentist can make a beautiful smile out of both traditional and ultra-thin porcelain veneers. However, a so-so cosmetic dentist will make a lot of artistic errors.

You asked how ultra-thin veneers can look bulky. I can answer that. There is a particular brand of ultra-thin veneers which is highly marketed to inexperienced cosmetic dentists as being easy to place because no tooth prep is required. While it may not be required, it is sometimes necessary. Not every smile does well without any tooth preparation and ends up with that bulky look you read about.

Composite filling hurts

I needed a dental filling on a back tooth. My dentist prefers placing the silver ones, but I didn’t really want that. I kind of pressured him to place the white one. Now I’m worried I made a mistake. The tooth hurts when I chew. I checked when I’m not eating and clenched, but it’s fine then. It only hurts when I’m biting down on something. Can this be fixed or am I stuck with it?

When it comes to choosing your filling, you did a great job. Composite fillings are much better than their amalgam counterparts. However, you need a dentist who is able to to place them properly.

Bonding a composite filling has a totally different technique than their silver counterparts. If not done properly, it can cause the pain you are describing.

This can be eliminated by having the crown re-done the right way. There is also an occasional problem with small composite fillings. We aren’t sure what causes it, but we do know if the dentist uses a self-etching primer or a glass ionomer base under the filling you won’t have pain.

You Need a Mercury-free Dentist

If you want this to be done properly, you’ll need a dentist to understand the procedure. I’m sure your dentist did the best he knew how, but at this point you need someone else, unless he’s willing to learn.

Either you or your dentist can contact a mercury free dentist who understands the right technique and can re-do your filling properly.

I hope this helps. It’s always better to find a different dentist for a procedure you have your heart set on in which your dentist is trying to steer you away from.

Usually, it means they don’t know this procedure as well, but they don’t want to admit it.

Do I Have tmj or tmd?

I’m a little confused about which problem I have. I’ve been having problems with severe headaches and pain in my jaw. Someone said they think I have TMJ . I went home to look that up and there seems to be two things. One is called TMJ and one is called TMD. How do I know which one I have?

There’s a good reason you are confused about this. Even though they have two different technical meanings, people (especially patients) use them synonymously.

TMJ refers to the temporomadibular joint. I’ve placed a picture of it above so you can see a close up of it. TMD refers to the disorder of the temporomandibular joint.

Generally, when people say TMJ they mean TMJ disorder (or TMD).

If You Have TMJ Disorder

The symptoms you’ve described above do relate to TMJ Disorder.
There are many possible reasons for developing jaw problems. Your next step would be to see a dentist with expertise in TMJ problems, so he can properly diagnose the cause.

You want a dentist who’ has invested in significant post-doctoral training in TMJ diagnosis and treatment. For instance, Dr. Delaune has done extensive training with the world renown Dawson Academy.

There is not a recognized TMJ Specialty so any general dentist can call themselves a TMJ dentist. That makes it especially important you know that the dentist you’re dealing with is actually qualified.

Treatments vary depending on the cause. It can range from a simple orthotic device up to a full-mouth reconstruction. Good dentists start with the most conservative treatment except for the most rare, devastating cases.

Lumineers versus DURAthin

I’m planning on a smile makeover. I’ve been looking at images of both Lumineers and DURAthin. I like the DURAthin a lot better based on what I’ve seen, but wondered about cost. Are the DURAthin much more expensive?

I’m concerned you are going about this the wrong way. It really isn’t the brand of porcelain veneers you get. It is the dentist. Let’s say two sculptors purchase the exact same type of marble to sculpt the same subject. It’s very likely one of them will have much more lifelike and beautiful work than the other. The skill of the artist truly matters.

The same is true for cosmetic dentistry. Whether you get Lumineers, DURAthin, or another brand won’t matter as much. I will say that most expert cosmetic dentists do not like the Lumineers brand. This is because they force dentists to use their lab which isn’t known for beautiful results.

What I’d like you to do instead of shopping brands is to shop dentists, but not by price. Instead, shop them by skill and artistry. Then, let them pick the best brand of porcelain veneers for the smile you want. They know the characteristics of each brand.

Worried Dentist Ruined My Porcelain Veneer

I just had a set of porcelain veneers placed. I did like them but noticed one of them had a scratch on it. I mentioned it to him and he said it’s no problem he could even fix it before I left. I was thrilled and he bonded them on. Then he did some something he called “buffing out the scratch”. It looked fine and I left happy. It’s been a couple of days and now that tooth he buffed looks different from the rest of them. It looks duller. I’m worried something is wrong with it. Am I overreacting?

No, you’re not overreacting. Your worry is quite founded in reality as well. It sounds like your dentist removed the scratch by removing the glazing. Not only is the glazing what gives your porcelain veneers their shine, but it’s also what protects them. Your veneers will quickly begin to pick up stains.

You need to have him replace this porcelain veneer. It’s best you call him right away. If you wait, he could try to say you did something to it. This way, if you call right away, he’ll have to admit it’s something he did.
