When Will I See Results from At-Home Teeth Whitening?

When will I see results from at-home teeth whitening? I started using carbamide peroxide 16% for my tetracycline stains about 5 weeks ago. I gave my teeth a break after only getting slight improvement. How long should I wait before trying again to avoid over-bleaching my teeth. I wonder whether I should try another brand of bleaching gel to see if the results are better. When should I see the improvement? – Thanks Zane

Zane – Understandably, you want to do something about the tetracycline stains on your teeth, but 16% carbamide peroxide won’t remove them.

When Will You See Results from At-Home Teeth Whitening?

However, some whitening methods are not effective at all. If you have tetracycline stains, bleaching gel may not help at all. If teeth bleaching will help the stains, it needs to be done professionally by a dentist with high-strength bleaching gel. Even if the stains don’t disappear, overbleaching your teeth can damage them. Only an experienced cosmetic dentist can determine the treatment method for tetracycline stains.

It takes time and skill to get good results, though. It isn’t something that you should try on your own. Depending on your teeth and the severity of the stains, teeth whitening might not be the best option.

Porcelain veneers are a natural-looking way to conceal the tetracycline stains on your teeth. However, covering them isn’t easy. It requires the skill of a cosmetic dentist to give you porcelain veneers to cover the stains without them looking pasty or opaque.

Find an experienced, credentialed cosmetic dentist to examine your teeth and recommend the right treatment.

Dental Bonding Doesn’t Match and Falls Off

I have tetracycline stains and my dentist said dental bonding would take care of it. It has not worked out at all like I wanted. First, the color doesn’t match and it is pretty obvious. Plus, you can still see some of the tetracycline stains through it. As if that weren’t bad enough, the bonding fell off after just three days. I went back for him to repair it. At the time, I told him about the color problem so he made them thicker, which looks weird in its own way. Now they have fallen off again just a few days later. I called him back and he said they’re not meant to be permanent so I should not have high expectations. However, I was told they’d last up to five years, this is like just a few days. What should I do?

As you can see from the image above dental bonding can blend in quite naturally with your teeth. I think the biggest problem you are facing is the skill and technical knowledge of your dentist. Both dental bonding and tetracycline stains are some of the most advanced procedures in cosmetic dentistry. You should be aware that cosmetic dentistry is not a recognized specialty. Any general dentist can call themselves a cosmetic dentist regardless of the amount of training they have invested (or not invested) in.

I think your dentist is in way over his head. Dental bonding should not fall off, especially after just a few days. As for the color, if he is an inexperienced cosmetic dentist, he probably doesn’t even have the colors and other materials necessary to make things blend well. An additional problem in dealing with tetracycline stains is making the teeth opaque enough to cover the stains, while translucent enough to look natural. That is not an easy accomplishment, even for a skilled cosmetic dentist.

Honestly, the better treatment for you would be porcelain veneers and not bonding. It is more permanent, decades rather than years. Plus, it is more stain-resistant. I don’t think your dentist is the one to do it. Here is my advice. Do not let him “fix” this. instead, ask for a refund. Then, we’re going to get you to a highly skilled cosmetic dentist.

Finding an Expert Cosmetic Dentist

Because there is not a recognized specialty in cosmetic dentistry it is hard for patients to know who has the skills and who doesn’t. They cannot purchase their way on. Instead, they have to show their training credentials, in addition to proving their artistry and skill on cases they’ve completed. Tetracycline Teeth and No Money

I have tetracycline stains on my teeth. I had dental bonding placed on my teeth but they are old and chipping. I can’t afford the porcelain veneers my dentist thinks I should get. Is there a whitening option that will work for me? Or if there are ways to get porcelain veneers cheaper than my dentist offers them can you give me that tip? My dentist is one of those top cosmetic dentists so I know he is more expensive than most.

I will answer your second question first. If you cannot afford to get porcelain veneers from a skilled porcelain veneers, please don’t go to a cheaper dentist. I can tell you countless horror stories of smile makeover disasters from patients who tried to save money by going to a cheap cosmetic dentist. This is especially true in your case because tetracycline stains are one of the most advanced types of cosmetic cases there are. If you cannot afford to have it done right you have a few options.

If your dentist placed the dental bonding over your dental enamel, you should be able to have your dentist remove the bonding with sandpaper disks. In some states, a dental hygienist can do this for you at a lower fee. Once the dental bonding is off, you can whiten your teeth. If you are persistant enough and stay at it long enough it can make a difference. It will not be a great as porcelain veneers but it will make a difference. Don’t use anything over the counter. They are not strong enough and it will be a profound waste of money. In that same vein, don’t use anything like laser whitening or Zoom whitening. Yes, they are stronger than the take home teeth whitening, but they are more expensive. You are trying to save money and it would take way too many sessions to make a difference.

On the other hand, if your dentist did remove some of the enamel on your teeth, the best you can do at this point is have someone smooth out the dental bonding while you save to get porcelain veneers done by a skilled cosmetic dentist.

Overbite and porcelain veneers

I have pretty bad tetracycline stains and wanted to get porcelain veneers to help cover them. My dentist said I am not a candidate for porcelain veneers because of my overbite. Instead, he wants to do dental crowns on my top six teeth. Is this a good alternate solution? Do I just bleach the bottom teeth?

Please don’t let your dentist do dental crowns. This will require him to grind your healthy teeth down to nubs. He is likely saying that because he doesn’t know how to do porcelain veneers. I’ll give him credit for having a creative excuse. I’ve never heard a dentist use an overbite as an excuse. In reality, an overbite is easier with porcelain veneers.

You can see from the image above that a tetracycline case can be made to look stunning. There is a big “HOWEVER” when you’re considering this though. Smile makeovers aren’t taught in dental school. Instead, a dentist has to invest in post-doctoral training in order to truly know how to create a beautiful smile. Now, factor in tetracycline stains. These are among the most difficult type of cosmetic cases to do.

What you need is an expert cosmetic dentist. The images above show a tetracycline case done by Dr. Delaune. You want to find a dentist with his training and skills in artistry.

Finding that Expert Cosmetic Dentist

One thing I will suggest is you look on the mynewsmile.com website. This site is run by a retired dentist who is accredited in cosmetic dentistry. He screens each dentist who wants to be listed for both their technical training as well as looking to see what type of beauty they create in the cases they do.

One thing to be aware of, I”m pretty sure whichever cosmetic dentist you end up with will suggest more than six porcelain veneers. Most of our smiles are at least eight teeth wide.

I’ve had tetracycline-stained teeth since I was a child. It has always been a source of horrible embarrassment for me. My dentist said teeth whitening wouldn’t do much good for me and not to waste my money. He didn’t have a solution for me either. Is there anything I can do or am I just going to have to learn to live with a smile I’d rather hide?

While one particular brand of teeth whitening has had more success with these stains than others, most make hardly any difference at all. That doesn’t mean all is lost.

As you can see from this smile makeover that Dr. Deluane did on a patient, there is a way to obtain a beautiful smile after tetracycline stains. Here Dr. Delaune used porcelain veneers. This enabled him not only to cover her stains, but to reshape her teeth to add length.

I don’t want you to think you can just run to your family dentist and get porcelain veneers and all will be beautiful and shiny. These are among the most difficult cosmetic dentistry cases to do. It would take a dentist with advanced cosmetic training.

The difficulty comes in needing to make the porcelain veneers opaque enough to cover the dark tetracycline stains, yet translucent enough to have their natural shine.

Porcelain veneers or crowns for tetracycline stains?

I’ve seen two different cosmetic dentists about dealing with my tetracycline stains. They’ve recommended two very different solutions. The first dentist, who is well within my budget, wants to do porcelain crowns on the visible teeth. The other one, who goes several thousand dollars over my intended budget, wants to do porcelain veneers. Is there anything wrong with doing the crowns?

Before we get into the cost difference, I’d like to talk about the different procedures. With dental crowns, you have to have a great deal of tooth structure removed. If those teeth don’t need that structure removed, you are weakening them unnecessarily.

Ask yourself why a dentist would choose to use a procedure which is unnecessarily aggressive. My guess is, it is the only procedure he’s comfortable with. Every dentist places dental crowns. That doesn’t mean they are skilled in cosmetic work.

With porcelain veneers, you only have to remove a minimum amount of tooth structure, about the thickness of a fingernail. You can always switch from veneers to crowns. But, once you have dental crowns, you will always have to have dental crowns.

Choose the Dentist Carefully

Tetracycline stains are among the most challenging cosmetic cases there are. Whatever is used needs to be opaque enough to cover the stains completely, but still be translucent enough to reflect light and look natural.

I would say only the top 1% of dentists in the country could do it beautifully. You need one of those dentists. I would recommend you look on the mynewsmile.com website. They screen every dentist who wants to be listed on their website.

No one can pay to be listed. They have to provide background on their cosmetic training as well as visual proof of their artistry.

One last thing, price doesn’t always give a good picture of the skill of the dentist. Some really bad dentists have high prices. Some really great dentists have average prices.

Tetracycline stains are nothing but trouble

I have some serious tetracycline stains on my front teeth and finally decided to do something about it. My dentist said that crowns would be best, but after having eight done (and re-done twice), I’m pretty much convinced he has no idea what he is doing. He replaced the first set because they looked yellow and their shape just didn’t look right. The second set was better, but the color was still off and they were too opaque. This last time he went back to the lab and had them blend in some translucent color in hopes they would look more natural. Unfortunately, when I got home and looked in the mirror, all I could see was gray streaks and spots. I’m thinking I should have just left my teeth the way they were. I have no idea what to now…any advice? — Kim

As you’ve probably discovered, tetracycline stains are some of the hardest to mask and most dentists are clueless when it comes to treating them. And unfortunately, it sounds like your dentist falls into that category. In fact, he probably had no experience with tetracycline stains (and neither did his lab) and the result was a complete disaster.

What your dentist should have recommended was porcelain veneers. Rather than grinding down your teeth, all that is required with veneers is shaving a little bit off the front of each tooth. And while porcelain veneers are the standard, there’s a fine balance that needs to be achieved to block out the stain and still get a natural look. If not done correctly, one of two things can happen: 1) the teeth will look gray from the stain showing through or 2) your smile will look like a porcelain sink.

Your first step in trying to get this corrected is to find a skilled cosmetic dentist in your area who can take a look. Then, you should pursue getting a full refund from your dentist and start completely over. Just be sure you find a cosmetic dentist who has experience with tetracycline stains to assure the best results possible.

Are veneers the only option to replace my bonding?

When I was younger, I took tetracycline and it stained my teeth. Back then, they did bonding to cover the stains and for the most part it has held up pretty well. It’s just been in the last few years that they have started to look bad again and I’ve been considering what to do about it. At my last dental appointment, I brought it up to my dentist and asked him what my options are. He insists that I can only do veneers – which are completely out of the question because there is no way I can afford them. I asked if we could do bonding again or if teeth whitening would work and he shot both of them down. Is there really nothing else that can be done? Will I have to just have dingy, stained teeth until I can save up enough money for veneers? — Marilee

Dear Marilee,
Do you know if your dentist does much cosmetic dentistry? Based on what he is telling, it doesn’t sound like he does. There’s no doubt that veneers would be the best way to give you a great looking smile, but there are other options.

Unless he’s examined you and determined that there are structural issues with your teeth, there shouldn’t be any reason why the bonding couldn’t be redone. If he’s pushing the veneers, your teeth are probably fine. As for whitening, tetracycline stains are highly resistant to even the strongest professional grade gels and it could take months of treatment to get where you want to be.

Here’s what you need to do: find a good cosmetic dentist and get a second opinion. He or she might be able to polish some of the stains out. If you do try whitening, you need to realize that it could take a long time to see any results, so you’ll have to be persistent and patient. The bonding won’t actually change color, so it will need to be replaced. A good cosmetic dentist can tell you whether or not the bonding can be removed before you start the whitening and then replaced after you’ve gotten the results you want.

This will be a lengthy process – probably several months at least. If you don’t think you can endure that long, you better start tucking away those extra dollars so you can eventually get veneers.

Will Lumineers work for tetracycline stains?

My husband has tetracycline stains on his teeth. We’ve looked into porcelain veneers, but they are so expensive. We saw a commercial for Lumineers and they are cheaper, will they work? — Sandy

For tetracycline stains, Lumineers would not be the best choice, and here’s why:

  1. Tetracycline stains are very difficult to cover. The veneer needs to wrap around the tooth or else the result will be a “halo” effect because the teeth will look whiter in the middle and gray around the edges. That requires tooth preparation and Lumineers are considered “no-prep.”
  2. Lumineers are not thick enough to adequately cover dark tetracycline stains. You’ll either have teeth that look a very faded shade of gray or they will be a pasty, opaque white that will not look like natural teeth at all.

Crowns or porcelain veneers for tetracycline stains?

I had porcelain veneers placed about 18 years ago to cover very severe tetracycline stains on my teeth. Even though the veneers looked gray, it was better than living with almost black teeth. Now my veneers need replaced, but when I consulted with my current dentist he and his lab tech decided on crowns instead.  I’m not sure this is the best choice… any advice?  — Ann

Dear Ann,
In this case, crowns would be a bad choice. Tell your dentist this is not an acceptable option and seek out a cosmetic dentist to get a second opinion. Since you’ve lived with your current veneers looking gray for all these years, you know that tetracycline stains are not easy to mask. But it sounds like your dentist (and the lab tech for that matter) simply don’t have the experience or skill needed for porcelain veneers – thus their reasoning for suggesting crowns. A true cosmetic dentist would not grind your teeth completely down.

If you want a beautiful, natural looking white smile, do some extra homework and find a  cosmetic dentist in your area. Even if you have to drive a few hours to find one, it will be worth the additional time and effort because you want to do this right.
